Gold plated pieces can usually be detected by close examination at the wear points; ie. connection points, where the metal links meets or near the connections & around clasp, wearing of the surface of the gold, peeling or flaking are usually easy to detect with old pieces.
If it's marked "ITALY" than pretty likely its been made in Italy but as the standard is 18 ct gold in Italy, it could have been made in Italy for the US market.
