What a nice idea, I'm sure your mom will love it. There are a couple different options. You could bring your idea (I would include sketches or photo's) to a custom jeweler. A lot of places take customer orders and make jewelry such as rings, pendants, necklaces and likely cameos. I'm not sure you will be able to fit everything you mentioned on a cameo as they are usually no larger then a half inch to an inch in height. Maybe getting a silhouette of you and your sibling back to back? I will leave the creativity up to you. Googling your town and "custom jewelry" will likely get you the results you seek. Good luck!
